I am a registered lobbyist/firm/employer. How do I notify the Lobbyist Division of a change to my information (i.e. new firm or company name, change of address, telephone number, responsible officer)?

You must complete an Amendment to Registration Statement (Form LOB 5) and file it with the Executive Officer of the Board of Supervisors.
